Brushing And Flossing Alone May Not Be Enough to Prevent Periodontal Disease

However, recent findings suggest that these practices alone might not be sufficient to prevent periodontal disease. A new article in Digital Dentistry & Healthcare Knowledge raises concerns about this traditional approach and suggests that additional measures are necessary.

The article, titled Peri-Implant Disease: What Your Patient Can Do to Prevent Losing the Implant; What You Can Do to Prevent Losing the Patient, highlights that periodontal and peri-implant diseases are still prevalent despite regular brushing and flossing. Herb Moskowitz, DDS, Chairman of ioTech International, stresses, “Daily brushing, flossing, and antibacterial rinsing are essential for combating these diseases.”

A six-month study involving over 300 dental patients revealed that combining brushing with rinsing was 18 times more effective than brushing and flossing alone. Another study found that brushing and rinsing were six times more effective in reducing dental plaque. Furthermore, an examination of 160 patients showed that brushing and rinsing significantly reduced plaque and inflammation, as measured by the Plaque Index, the Modified Gingival Index, and the Bleeding Index.

Moskowitz emphasizes the importance of including rinsing in daily oral care. He points out that home gingival irrigation with an oral irrigator or water flosser can greatly improve oral health. Rinsing alone can reach 21% of a 6mm pocket, while using an oral irrigator can reach up to 90% of the same pocket.

“Effective rinsing and subgingival irrigation are crucial for preventing periodontal and peri-implant diseases,” Moskowitz says. “Not all oral rinses are created equal.” He notes that molecular iodine has proven to be more effective than other professional rinses in maintaining gum health.

The article compares various antiseptic rinses against key bacterial pathogens, including Fusobacterium nucleatum and Prevotella. Both are linked to periodontal disease, with Fusobacterium also associated with colon cancer. The results showed a significant variation in efficacy among the tested rinses. Molecular iodine was found to completely inactivate both bacteria, while other rinses, such as stabilized chlorine dioxide, showed little to no effectiveness. It disrupts pathogen cell walls, leading to rapid microbial death and reduced biofilm formation on implant surfaces. Moskowitz advocates for integrating molecular iodine into regular oral hygiene practices to extend the life of dental implants and improve overall oral health. He notes that up to 75% of U.S. adults have some form of gum disease, often unnoticed until severe issues arise. Alarmingly, nearly 50% of analyzed dental implants have developed peri-implant disease, which could have been prevented with proper home oral hygiene and an effective antibacterial rinse.
