Orthodontic-Restorative Interface: A Contemporary International Perspective

by Mary

I am pleased to present this Themed Issue focused on the Orthodontic-Restorative Interface. The ongoing advancements in orthodontic appliances and diagnostics, combined with a growing recognition of their relevance to adult treatment, signify a new era in orthodontic care. Orthodontics, once primarily aimed at adolescents, is now increasingly accessible to adults.

This shift offers significant opportunities for comprehensive orthodontic correction, providing both functional and aesthetic benefits.

However, adult orthodontics can be complex due to factors such as aging, dental issues, and previous treatments. It is crucial for dentists and orthodontists to maintain a holistic view when addressing the various challenges presented by adults with malocclusion.

The rise of adult orthodontics has coincided with improvements in digital techniques and the broader use of aligners. While I understand the advantages of the tools available, the intricate problems faced by many adult patients require more than just technical solutions. They demand a thorough diagnostic approach, an understanding of the biological effects of our treatments, and an awareness of the limitations associated with single-specialty care.

In this issue, I have emphasized the importance of diagnostics and theoretical knowledge. This includes the orthodontic implications of previously traumatized teeth, restorative work, and periodontal disease. I have also included case studies that demonstrate successful clinical outcomes, even in complex situations.

Our goal is for these articles to highlight the necessity of foundational knowledge in achieving reliable results in challenging clinical scenarios.

It is important to recognize that adult patients often have a history of dental disease or trauma. This reality underscores the need for interdisciplinary approaches. Some patients may require more extensive interventions to address these complications, while others might benefit from a more conservative strategy. Clearly, a one-size-fits-all approach is not feasible.

We aim to provide a collection of articles that showcase both limited-objective treatments for those not suited for comprehensive care and examples of interdisciplinary excellence. This will illustrate the value of well-planned, orthodontically-driven combined care for adults.

I am grateful to the leading clinicians and researchers who have generously shared their expertise in this Themed Issue. We have invited respected national and international professionals to contribute, and I sincerely appreciate their input. It is my hope that the articles inspire you by demonstrating what is achievable through well-coordinated treatment by skilled specialist teams.

These contributions may influence your objectives in complex joint cases, encourage you to collaborate more readily with specialist colleagues, or ignite a passion for learning more about related specialties to enhance patient care.

For instance, consider the importance of focusing on the width of narrow central incisors rather than canines during canine substitution for missing lateral incisors. Additionally, one might question the likelihood of effective gingival movement during orthodontic extrusion of a tooth with a thin periodontal phenotype.

Adult orthodontics and combined orthodontic-restorative care have firmly established their place in dental practice. We are likely to see an increase in this type of work in the future. I hope you find this issue enjoyable and that you appreciate the valuable contributions from the authors. This series aims to serve as a useful foundation for your ongoing education in an evolving and exciting field.
